REGISTRATION AGREEMENT

ENROLLMENT POLICY:  Enrollment is accepted on a first-come – first-serve basis upon receipt of signed registration agreement, form, and payment of registration fees.

Classes at maximum enrollment will be closed to additional students. Classes that do not meet the minimum requirement for students will be cancelled. If a class is cancelled, students registered will be offered an alternate class. If the student is unable to attend an alternate class, a refund for tuition will be offered.

TUITION POLICY: (Please circle choice of payment plan.)

Option one – Five equal payments due every other month. (First payment due at registration, last payment due in June.)

Option two – Two equal semester payments. (First payment due at registration, Second in January.)

Option three – One full semester payment. (Full payment due at registration.)

Cash or checks are acceptable forms of payment. Checks can be made payable to "AHAdance."

Note: The first payment and a $25.00 non-refundable registration fee are due with this signed form and registration form in order for registration to be processed.

Tuition is based on classes for an entire season (October through June). There are no refunds or credits given for absences or cancelled classes due to holidays or inclement weather. Attendance in class is important to the learning process. Absenteeism should be avoided. Absences can be made up in equivalent classes only when available, but should not be considered a substitute for regular classes that are missed.

WAIVER:

I understand the activities and risks involved in dance classes. I hold harmless AHA! Amber Hemmer’s Academy of Dance, LLC, including all employees and representatives from all liability, suits, and claims arising from participation in or observation of any AHA! Amber Hemmer’s Academy of Dance, LLC activities or events. AHA! Amber Hemmer’s Academy of Dance, LLC is not responsible for any lost, stolen, or damaged property. I also recognize that AHA! Amber Hemmer’s Academy of Dance, LLC is not responsible for students when they are outside of the classroom.
